Section B is required to be signed and duly stamped by the Police Station or Notary Public to confirm certification.<\/li>\n<li>Sample Passport Application Form for Minors\u00a0<\/li>\n<li>Sample\u00a0Passport Application Form for Adults<\/li>\n<li><strong>DHA-9 form \u2013 Full set of fingerprints on DHA-9 form<\/strong>. This form is not downloadable. Please send a self-addressed stamped envelope with a request for DHA-9 to receive this form. This form must be submitted by applicants 16 years old and above. Fingerprints can be taken at the nearest police station, fingerprints taking centers, or a notary public office. Electronic fingerprints on police cards are not accepted.<\/li>\n<li>See sample DHA-9 form.<\/li>\n<li>The written motivation for the issuing of the Emergency Travel Certificate, along with any proof of the urgency of the trip (e.g. confirmation of funeral arrangements, doctor&#8217;s letter confirming sickness, police report, etc.)<\/li>\n<li>A schedule of your travel arrangements (flight itinerary)<\/li>\n<li>Proof of your South African citizenship in the form of your South African identity book, birth certificate or old passport and a copy thereof<\/li>\n<li><strong>DHA-529<\/strong>\u2013\u00a0<strong>Determination of citizenship<\/strong>. This form must be completed by applicants who are 16 years old and above;<\/li>\n<li>See sample DHA-529 form.<\/li>\n<li><strong>Divorce decree<\/strong>. To be submitted by all divorced applicants;<\/li>\n<li><strong>Notarized Marriage Certificate<\/strong>. To be submitted by married applicants irrespective of whether they will or will not change their surname;<\/li>\n<li><strong>Written Request to Change &#8220;Surname&#8221;<\/strong>. To be submitted by married female applicants who would like:\n<ul>\n<li>1) to retain their maiden surnames, or<\/li>\n<li>2) wish to use double-barrel surnames;<\/li>\n<\/ul>\n<\/li>\n<li><strong>Notarized Copy of South African Passport<\/strong><\/li>\n<li><strong>Notarized Copy of South African Identity Book or Birth certificate<\/strong>;<\/li>\n<li><strong>Notarized Copy of Retention\/Exemption Letter<\/strong>, for those who have dual citizenship;<\/li>\n<li><strong>Notarized certificate of Naturalization<\/strong>. To be submitted by applicants who are naturalized South Africans.<\/li>\n<li><strong>Loss of Passport Report\u00a0and Police Report for Loss of Passpor<\/strong>t. To be submitted by applicants who lost their passports and are applying for a new passport;<\/li>\n<li><strong>Four (4) passport size photos<\/strong> (2&#8243; x 2&#8243;) Please note that color photos that comply with the\u00a0International Civil Aviation Organization (ICAO) standards for passports photo images\u00a0are required for South African passport applications. [The holder&#8217;s signature (persons 16 years old and above) is laser engraved into the passport. To ensure good quality scanned signature for laser engraving into the passport, the signature in the signature box on the passport application form should be in the middle of the box and must not touch the sides of the box. A medium point ballpoint pen should be used to sign the box]. Please ensure that your mouth is closed and your teeth are not showing on the photo.
